Payment Options

    We believe in the value of chiropractic care for you and your family and have maintained a pricing structure that allows care for ALL budgets. Our office is able to participate in most insurance programs. We also accept auto accident and workers compensation cases.

     For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. Realizing no one wants to build up a large bill we will work with you so that you and your family can receive complete care without undue financial difficulty. Of course we are always happy to accept cash, check, debit or credit card. To learn more about our payment options for chiropractic and wellness care, you can call our office directly.


We'll happily telephone your insurance company to verify your coverage benefits. Please remember, however, that the benefits quoted to us by your insurance company are not a guarantee of payment. We will file your claim and await payment for 90 days. If your company pays the benefits they quote to us, the only services you will be responsible for are any non-covered services, deductibles or co-pays. If you have any questions at all regarding you financial responsibilities at our office, please do not hesitate to ask.

For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. If you have no insurance or do not have chiropractic benefits there is still a way for you to receive the care you need. Many patients pay directly for care, as they discover chiropractic to be extremely cost-effective and affordable.

"ON THE JOB" INJURY (Worker's Compensation)

If you are injured on the job, your care should be paid for under your employer's Worker's Compensation insurance. You will need to inform your employer of the accident and obtain the name and address of the carrier of their insurance.


Please notify your auto insurance carrier of your visit to our office immediately. Notify us if an attorney is representing you. Although you are ultimately responsible for your bill, we will wait for settlement of your claim for up to six months after your care is completed. Once the claim is settled or if you suspend or terminate care, any fees for services are due immediately.


We do accept assignment from Medicare and most Medicaid policies. We will bill Medicare or Medicaid directly for you. Medicare pays 80% of the allowable fee once the deductible has been met. However, Medicare will only cover manual manipulation of the spine for chiropractic. Therefore, any other procedure done in the office will not be covered.

At Sazama Family Chiropractic & Wellness, P.A. we're convinced that a small amount of your time and money to care for your most important asset--your health--will be far less time consuming and expensive than trying to recover it. If you feel you do not have enough time or money to take care of yourself, please contact us--we can help! The last thing we want is for you to come to our wellness center in crisis.
